Glee’s Cory Monteith died of a heroin and champagne overdose reveals the final coroner’s report.

Cory Monteith’s overdose on 13 July has been revealed, and it paints a tragic picture for the young star.

The report – which has been obtained by TMZ – states that the 31-year-old actor was shooting heroin and downing champagne alone in his hotel room on the night of his death.

According to the report, first responders discovered a “spoon with drug residue and a used hypodermic needle.”

Two empty champagne bottles and glasses were also discovered at the scene.

The young star’s official cause of death is listed as “mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol.”

It’s also noted that Cory “had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs.”

Cory’s tolerance for substance abuse may have been lower than usual because he had stopped using since attending rehab over a period from March to April, the coroner posits.

At the time police Constable Brian Montague said: “There was evidence in the room that was consistent of a drug overdose,’ said Constable Brian Montague, adding that he wouldn’t ‘go into specifics.”

“Mr. Monteith was alone [in his hotel room] when he died,” a Vancouver Police Department spokesperson said.

“He took the heroin when he was alone by himself.”

It is still unclear what happened to Monteith in the hours leading up to his death.

He and his manager dined on the Thursday night before his death with Maureen Webb, the co-founder of Project Limelight Society, who said the actor ‘looked so good, so healthy’.

According to authorities, Monteith and three unidentified people left his hotel room on Friday night and that the actor returned alone at around 2:15 a.m. Hotel staff found his body shortly after noon when Monteith didn’t check out.
